e-brake adjusting mechanism not working!

Fully taken appart.
 I went to my local garage and we took a look at it, but noone could understand how this silly thing works. Anybody know if it adjusts by the e-brake leaver or by hydraulic pressure? Going by the parts it can´t be triggered by the leaver, cause its fu**ing fixed on both sides of the parts with thread!!! Does anybody know, if threaded parts in the piston is supposed turn in opposide direction of the thread?
