As I see you had a bad pinion height set up! The teeth didn't contact at the "middle" And other idea is you didn't use solid spacers, the third is just an additive, there was not enaugh oil near the teeth - maybe you drifted (or tracked?) long time at one direction and the oil went away.

We used to over fill the rear axle and upgrade the breathing it helps a lot for the final gears.
