No, no such thing as worn out until it breaks or the pads aren't conforming with the disks concentric grooves.

Maybe. Adjusted right & all pivot points & linkage & cables lubed?
3. Pistons are not working hard enough in the calipers
Maybe. pistons could be frozen/corroded in caliper.
4. Bad brake pads
No. Good pads for an easy handbrake would be too soft & lockup easily when using the hydraulics & overheat quickly.
